CDS Life Transitions Direct Support Professional- Community Habilitation in Rochester, New York
Summary:
Community Habilitation supports the person's Life Plan with learning, building and gaining independence to live more independently, to achieve their goals and to be more involved within the community. Services are provided in the home and/or while out in the community
Essential Job Functions:
Act as mandated reporter if there is reasonable suspicion to suspect abuse or neglect of Individuals and make all required reports in established timeframes, as required by the NYS Protection of People with Special Needs Act.
Maintain all required trainings (classroom and online) relevant to the position.
Provide supports to individuals with developmental disabilities to help them achieve personal outcomes and growth.
Provide supports by teaching daily living skills as outlined in the plans such as:
Self-care skills
Household chores/Cooking
Personal health care
Money management skills
Social skills
Fire evacuation
Learning methods of transportation
Self-advocacy
Community inclusion/Relationship Building
As well as any other skills identified in the person’s staff action plan.
Provide face to face supports to individuals by assisting in the development and implementation of all support plans.
Assist individuals in achieving their personal goals and desires.
Provide interaction and choices for activities.
Support individuals to be independent in activities of daily living.
Partner with individuals to promote their health and safety.
Promote positive behavior and supports through partnership with individuals, families and circle of supports.
Complete all documentation related to daily services provided within required guidelines, including monthly summaries.
Comply with all OPWDD, DOH regulations and CDS policies and procedures.
Demonstrate respect for individuals, diversity, and inclusion.
Perform all other duties as assigned.
Meet with the assigned individual as scheduled based off approved OPWDD units.
Transport individuals in an Agency vehicle.
Knowledge, Skills, and Abilities
Ability to work independently and as a team and motivate others.
Ability to read and write English.
Ability to communicate effectively, both orally and in writing.
Ability to manage time effectively
Ability to drive throughout the duration of a shift
Ability to be flexible and work varying hours depending on the need of the individual.
Education and Experience:
High School Diploma or equivalency
All experience and education requirements, except when required by federal, state, or local laws or requirements, may be waived at the discretion of management with the approval of the Executive Vice President, in collaboration with Human Resources.
Physical Requirements/Working Conditions:
Must be able to push/pull up to 25 lbs.
Must be able to stand, walk, and stoop frequently.
May occasionally kneel or crouch. Kneeling is defined as bending legs at knees to come to rest on knee(s). Must be able to kneel in emergency situations to administer CPR. Crouching is defined as bending body downward and forward by bending spine and legs.
Corporate Qualifications/Expectations:
Must be at least 18 years of age
Must meet applicable Vehicle Operator Requirements. (NYS Driver’s License required)
Adhere to all CDS Life Transitions, Inc. policies and procedures.
Adhere to the Agency Mission, Vision, Shared Values, and Customer Service Standards.
Attend mandatory education and training modules as scheduled; obtain and maintain all required certifications/training by State regulations and CDS policy
Act as a professional representative of CDS Life Transitions, Inc. in regards to appearance, behavior, temperament, communication, language, and dress.
