Job Title: Family Liaison

Location: Rochester, NY

Position Type: Part-Time (24 Hours per week)

Shifts: Days, Evenings & Weekends, as needed

Responsibilities:

As a Family Liaison, you'll provide essential support to parents and individuals within the

Community Services Community Habilitation Program. Key duties include:

  • Care & Support: Assist families in navigating and coordinating services, ensuring that needs are met in alignment with program goals.

  • Collaboration: Work closely with families, service providers, and community partners to ensure seamless communication and access to available resources.

  • Self-Directed & Co-Managed Services: Receive training and instructions from individuals or their designees, ensuring services are provided according to personalized guidelines and protocols.

  • Advocacy: Help families understand and access services, offering guidance and support to ensure informed decision-making.

  • Documentation: Maintain accurate and up-to-date records of services, communications, and progress, meeting program and regulatory standards.

  • Empowerment: Support families in understanding their rights and options, promoting independence and informed decision-making within the program.

  • Community Engagement: Help families connect with community resources and foster a network of support.

  • Additional Support: Attend training, meetings, and workshops to stay informed on best practices and program developments.

Qualifications:

  • Bachelor's degree in field of education, social work, psychology, rehabilitation, or

related human service field from an accredited program and 2 years of experience

serving individuals with developmental disabilities with a minimum of 2 additional

years working as a case manager in the MH or DD field. OR:

  • Associate's degree in human services and 2 years of experience serving individuals

with developmental disabilities and a minimum of 3 years' experience working as a

case manager in the MH or DD field.

  • Must have a valid New York State Driver's License and meet Lifetime Assistance Vehicle Operator Requirements
