As a Social Worker Housing Coordinator, you play an important and trusted role in the lives of patients and their family. You are an integral part of their care and recovery as well as a team player.
STATUS: Full Time
LOCATION: ElderONE- Hudson
DEPARTMENT: Social Work
SCHEDULE: Monday-Friday, Days
ATTRIBUTES
- BSW required, MSW preferred
- One (1) year of experience in a healthcare setting preferred
- Compassionate, warm and patient focused
- Exceptional documentation and planning skills
- Excellent communication and interpersonal skills
- Discharge. Partners with the assigned SW and RN Care Manager to ensure barriers to discharges related to housing and other social determinants of health are addressed
- Assessment. Assesses for risk of homelessness or disruption in housing situation and creates plans to manage. Manages and creates "bed board" for transisiontal housing; coordinates the utilization and length of stay with the IDT's
- Treatment Plan Development. Creates alternatives to SNF and inpatient by leveraging RRH assets and creating new relationships and opportunities for temporary and short term housing
- BS: Social Work (Required)
LICENSES/CERTIFICATIONS:
- LMSW - Licensed Master Social Worker - New York State Education Department (NYSED)
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
