The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

Provides professional social work services to patients and their families; collaborates with hospital staff and community social services resources relative to the psycho-social problems of patients; participates in formulating procedures and policies of the Division of Social Service; and participates in educational and research programs, including the orientation and training of new staff and students.Responsibilities General Purpose: The Social Worker assigned functions as a Team Lead for the service and is responsible for the coordination of continuity of care and discharge planning on that unit. The Team Lead would collaborate in supporting the general oversight and supervision of social work services for Child & Adolescent Psychiatry Inpatient as appropriate. May assist and provide consultation around complex discharge planning for GCH Inpatient social workers, as appropriate. The social worker who occupies this position provides social work services to patients and their families admitted for mental health treatment. As a member of the interdisciplinary treatment team, the social worker participates in the development and implementation of the patient's treatment and discharge plan. The social worker is responsible for providing psychosocial and family assessments, and for the overall coordination of the discharge planning process for patients primarily hospitalized as a part of the Rapid Stabilization Pathway with back up support to Child and Adolescent Inpatient Psychiatry unit, 49000. The social worker provides individual, family, or group treatment that will contribute to the implementation of a comprehensive discharge plan. The social worker also provides social work treatment in response to patient/family psychosocial problems which may impede care and/or discharge. The social worker develops interagency relationships and mobilizes community resources on behalf of the patients and/or their families. Responsibilities: - Functions as Team Leader and may assist in providing clinical supervision and contribute to the evaluation of an interdisciplinary group of two staff. Take the lead on SW internship experience on child inpatient unit, with the presence of at least one student. - Assists in recruitment, training, and evaluation of Inpatient social work staff in collaboration with appropriate interdisciplinary members of the program. May assist in supporting weekly meetings of the inpatient social work staff, which may consist of either informal/formal individual supervision or within treatment team settings. - Functions as a member of an interdisciplinary team, primarily within Rapid Stabilization Pathway and participates in patient rounding. Complete Psychosocial Risk Screen at time of admission and ongoing assessment throughout hospitalization. Provides individual, family and group interventions in response to identified psychosocial risks. Primary responsibility to the 4 beds on 39100. - Documents these interventions in compliance with Division, Department and Hospital Policy, to include any safety plans or other documentation crucial to discharge planning and continuity of care. - Identifies and coordinates safe and timely discharge plan with treatment team, patients/families and ensures a comprehensive discharge plan. Supports the safe discharge. Identifies contacts and coordinates with outpatient providers/agencies to facilitate continuity of care and discharge planning for the patient. Assess agency involvement and mobilizes family and/or appropriate community resources to support the patient's successful functioning post discharge. - Collaborates regularly with the C&A Inpatient Leadership Team in the identification and resolution of problems within the delivery of care system with special emphasis on discharge planning for patients on the C&A Inpatient Service. Collaborates regularly with the C&A Leadership for the purpose of needs assessment, program development, operations monitoring and quality assurance. May assist in providing consultation support to Golisano Children's Hospital social work or CL team around complex discharge planning. - Develops administrative liaisons with community agencies including but not limited to OMH, CPS, SPOA, OPWDD and referral resources to facilitate patient movement from the inpatient service to other locations, including home and/or community. Participates in regularly scheduled meetings with community agencies as scheduled for community planning and support. - Provides education to patients and families related to internal and community resources and referrals, advocates and supports patients and their families in their efforts to interact with complex social service agencies involved in their care. - Attends Social Work Division and Department Staff meetings, as required. Participates in activities of the Social Work Division including, but not limited to timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews. - Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competence events and training appropriate to job duties. - Provides coverage for vacations, sick calls, as needed. Other duties as assigned. Qualifications: - Master's degree from an accredited school of social work (CSWE) required - 3-4 years professional social work practice beyond the Master's degree, at least one of which must include psychiatric emergency, crisis or outreach work; or an equivalent combination of education and experience and working with pediatric populations preferred - Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions preferred; - Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families preferred; - Dedication to delivering a team-based integrated model of care preferred. - Demonstrates cultural humility and anti-racism values and works toward ensuring all patients feel welcome preferred. - Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required. If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start.
