Compensation Range:
$57,595.20 - $80,600.00
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
Provides professional social work services to patients and their families; collaborates with hospital staff and community social services resources relative to the psycho-social problems of patients; participates in formulating procedures and policies of the Division of Social Service; and participates in educational and research programs, including the orientation and training of new staff and students.Responsibilities General Purpose: The Social Worker is responsible for providing clinical services to pediatric patients and their families receiving care at the ambulatory Pediatric Orthopedic Practice. This includes assessment of barriers to orthopedic care and psychosocial risks, provision of crisis and ongoing intervention(s) to enhance orthopedic treatment outcomes and manage psychosocial risk situations, provision of care management and care coordination activities including pre -hospitalization and post -hospitalization needs, and referral to Hospital and community resources. Responsibilities: - Provides social work risk evaluation and assessment for patients identified as being at psychosocial risk (possible child maltreatment) or who have issues with non - compliance and inappropriate utilization of orthopedic care. In collaboration with the orthopedic team and patient/family, devises a plan for addressing risks and removing barriers to care. - Provides pre-admission planning for patients, helping to assure transportation, lodging, appropriate guardian consent and consideration of post - hospital care needs. Provides supportive and educational counseling to patients and families to support their ability to benefit from treatment, prepare for hospitalization and/or rehabilitation. Attends discharge planning meetings to facilitate transition to post hospital care. - Identifies risk management issues, communicating these issues to the Pediatric Clinical Manager. Assists in the identification of and assumes primary responsibility for managing all cases of suspected child abuse and neglect in the identified patient population - Provides concrete services to families needing assistance with transportation, car seats and other concrete resources to facilitate the outpatient visit. - Provides financial counseling for patient and families around special Medicaid programs, for supplemental medical insurance, and medical care at home. - Identifies patients eligible for Children's Health Home services and facilitates referral with parental consent - Serves as resource consultant to patient's families. Collaborates with schools, day care, and other community agencies serving the child. - Collaborates with other social workers at Golisano Children's Hospital to ensure continuity of social work care between other specialty ambulatory and inpatient settings. Documents social work activity in patient's medical record in accordance Social Work Division Policy and Procedure. - Acquires knowledge of and continues to receive professional development on complex medical conditions requiring orthopedic procedures and the impact on functionality and child's care. - Participates in activities of the Social Work Division including, but not limited to timely completion of e - record time spent for statistical reporting, absence approval and other Hospital or Division forms, Hospital and Division mandatory training and UHS review.
