Transition Services Coordinator I

MINIMUM QUALIFICATIONS:

  • High School Diploma or GED.
  • Four or more years of experience in education and/or working with individuals with disabilities (IWD).
  • An equivalent combination of education, certification, training, and/or experience may satisfy the above requirements.
  • Florida Department of Children and Families (DCF) Level 2 background screening requirements.
  • Must have first aid, cardiopulmonary resuscitation (CPR), and automated external defibrillator (AED) certifications and eight hours of continuing competence units (CCUs) as specified by the Agency for Persons with Disabilities (APD).
  • May be required to have or obtain additional formal industry certification(s) based on area of assignment.
  • Pre-employment drug screening.

PREFERRED QUALIFICATIONS:

  • Bachelors Degree in related field.
  • Experience working with IWD.
  • Experienced in Behavioral Management.

In addition to meeting the minimum qualifications listed above, an individual must be able to perform each of the established essential functions in order to perform this job successfully.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of the principles, practices, techniques and professional standards in the field of Habilitation services for individuals with disabilities (IWD).
  • Knowledge of community organizations and social service programs.
  • Knowledge of principles and procedures for employee support services.
  • Knowledge of the Health Insurance Portability and Accountability Act (HIPAA).
  • Operate a computer using Microsoft Office suite and applicable department/organization specific software.
  • Works with confidential employment information.
  • Analyze, and interpret program related laws and regulations and establish necessary work processes and procedures.
  • Manage multiple priorities to ensure that deadlines are met.
  • Use critical thinking skills to arrive at solutions and suggest improvements to processes.
  • Establish and maintain and collaborative working relationships with those contacted in the course of work.
  • Employer has attendance standards.
  • Communicate orally and in writing.

JOB SUMMARY:

Coordinates Transition Services Training program activities for BAC participants to include goal planning, work assignments, progress reports, and communications to Individuals with Disabilities (IWD) to allow them the opportunity to grow and learn within the work environment. Schedules day to day work activities/lessons to provide optimum efficiency and productivity. Work is performed independently under limited supervision with moderate latitude in the use of initiative and independent judgment. Position typically requires processing and interpreting of more complex, less clearly defined issues.

Provides various Transition services to participants based on area of assignment, individual skill level and in accordance with Transition team plan.

Assists with program planning and implementation activities; monitors and evaluates effectiveness; suggests improvements as needed.

Maintains documentation on individual participant objectives and progress towards goals; submits documentation to program administrators and other authorized recipients, as scheduled.

Serves as liaison between participants and family/community members to ensure accurate and timely relay of information.

Works with transition services instructors to develop training methods and monitor participant goals.
