DESCRIPTION OF WORK As an Aging Care Manager 1, you will be required to have a thorough working knowledge of the programs policies and procedures as you will be guiding individuals, groups, agencies, and staff through operations. You will be tasked with managing a caseload, working with consumers and family members, as well as developing care plans. You will have the opportunity to work closely with other agencies, hospitals, clinics, courts, and community resources in planning to meet the needs of consumers. Work includes participating in meetings and trainings, maintaining updated files, compiling reports, and gathering information from consumers.

Upon successful completion of six month probationary period, Aging Care Manager 1 will be promoted to a Aging Care Manager 2 per union agreement.

Work Schedule and Additional Information: Full-time employment Travel will be required. Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch. QUALIFICATIONS Minimum Experience and Training Requirements: Three years as an Aging Case Aide 2; or A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.

Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree. Hiring preference for this vacancy may be given to candidates who live within Potter County. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance. This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
