Community Care Network is looking for a School Based Clinician to join our team!

Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service.

About the Position:

The School Based Clinician is responsible for the provision of diagnosis and evaluation, psychotherapy, community supports, service planning, and coordination to all children who are needing additional mental health supports. Those services can include crisis interventions, trauma informed supports and resources, which are embedded directly into the child\'s school and therefore more easily accessed.

Responsibilities:

Provide clinical assessments and interventions for students and families.

Maintain case rate expectations which for the school year (PBIS school) consists of 9 Medicaid eligible students being seen for support/services for 2 hours per month. For a (non PBIS school) those expectations are 10 Medicaid eligible students being seen for support/services for 2 hours per month.

Provide individual, group and family therapy, based on emotional need, maintaining a full case load, based on mental health standards and ethics.

Enhance awareness of mental health needs, services and to promote community understanding and response to mental health problems.

Make home visits to support families. Facilitate communication between agencies on behalf of the client. Attend team meetings to plan and monitor services. Provide case management, collateral contact and social support, as needed.

Provides consultation services to teachers, community partners, and school personnel, as needed. Consultation can be educational or evaluative. Also provides consultation and support to staff.

Qualifications:

Master\'s Degree in Psychology, Counseling or Social Work required, Vermont State licensed or working towards.

2-3 years\' experience providing mental health services for diverse range of school-aged children living with serious emotional disorders required.

Proficient using computer and necessary software as well as experience with navigating an Electronic Medical Record to complete client documentation.

General knowledge of community resources, standard models of service coordination, and/or experience working in a variety of human services settings.

Handles stressful situations in a calm and professional manner.

Must have a valid driver\'s license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.

*Successful completion of a criminal history background check after hire.
