Job Summary
Provides preventive screening services, education, and outreach to vulnerable populations. This is an opportunity to contribute to the wellness of local communities. This position will work closely with the Ryan White Part B Program. Providing support and assisting with insurance, benefits and eligibility needs of the clinic as well as determining eligibility needs associated with the Ryan White Program. Working in an interdisciplinary team including medical outreach case management, medical providers, and supportive staff to provide the best care possible to clinic patients. Other administrative tasks will be assigned as needed.
Responsibilities
Link clients to services to help link to medical care/medications
Provide health counseling and education to increase knowledge of self-sufficiency.
Work with an interdisciplinary team in a fast-paced clinic setting to coordinate client care.
Helping clients with HIV education and understanding health needs.
Provide outreach to potential and existing clients in the community.
Assist in the coordination of client referrals to housing, food, transportation, and other social services as needed.
Work with staff and community agencies to establish trusting relationships with clients while providing support and encouragement.
Fill out client electronic intake questionnaire and screening assessment.
Make regular follow-up calls and assist patients with follow-up appointments.
Contact insurance companies to verify client coverage
Minimum Qualifications
High School diploma, or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated relationship-building and effective communication skills.
This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.
