ADEPT HOME HEALTH CARE INC Caregiver in SAN ANTONIO, Texas

**Job Title: Companion/Home Health Caregiver Reports to: Field Supervisor

We are currently looking for multiple individuals to add to our growing team. We work one-on-one with clients in their place of residence, and help them with their everyday lives. Every client has their individual care plans we follow, and we have clients throughout San Antonio and the surrounding counties. The ideal candidates are reliable, helpful and have a passion for helping other.

Scope of Position:** Responsible to perform personal care tasks as per definitions at 40 TAC 97.2 personal care tasks which may include bathing, dressing, grooming, feeding, exercising, toileting, positioning, and assist with self-administer medications, routine hair/skin care, and transfer/ambulation and /or other housekeeping related tasks as authorized by Texas Department of Human Services. The attendant is responsible to work the hours authorized by Texas Health and Human Services/HMO. Tasks to be performed by a home care provider shall be assigned by and performed under the direction of a supervisor or registered nurse that shall be responsible for the client care. Under no circumstances will the attendant/provider perform invasive procedures, wound care, injections and/or med administration unless specifically instructed to do so by the supervising RN as directed by delegation criteria. Do not perform any duties that are not authorized in writing by your supervisor, moreover any hours worked as unauthorized overtime will not be paid, and it will lead to job separation. Moreover, all hours unreported properly via EVV system or unverified due to not adhering to / following the designated schedule won't be considered payable working hours. Any overtime hours should be pre-authorized in writing.

Experience: Experience preferred, not required. CNA certification not required. Experience includes: Assisting individuals with personal hygiene care, grooming, bathing, transferring clients, standby assistance, and more.

Qualifications: 1. Must be 18 years older. 2. Demonstrate competency in providing personal care to clients. 3. Observes and practices rules of conduct, ethics and confidentially. 4. Must have reliable transportation.

Physical requirements: Work capability is variable, requiring a wide variety of body movements in actively working with clients depending on the client's individual needs and authorized service plan and hours; Sight and hearing sufficient to comprehend written and verbal communication and instruction; Ability to work a minimum of (8) eight hours per day while performing duties that require standing, bending, lifting, sitting; be able to work closely with others and adhere to irregular work schedule hours; Ability to work in by standard and/or unsanitary housing conditions occasionally required.

Duties and Responsibilities include but are not limited to: 1. Assisting with activities of daily living such as bathing, dressing, feeding, preparing meals, transferring and toileting. 2. Assisting with personal maintenance, such as grooming, bathing, dressing, routine hair and skin care. 3. Assisting with general household activities and chore services which are necessary to maintain the home in a clean, sanitary and safe environment , such as changing bed linens, house cleaning, and laundry, shopping, storing purchased items and washing dishes. 4. Providing protective supervision as temporary relief of the primary caregiver. 5. Providing ambulatory and exercise. 6. Assisting with medications that are normally self-administered (only reminding, opening med bottles or med box for patient to remove and take own medication. 7. Performing nursing tasks only when delegated by registered nurses. 8. Escorting the participant on trips to obtain medical diagnosis, treatment or both. The attendant/homemaker may not transport client in clients vehicle or in their own vehicle* 9.Reports to work at the client' home on the assigned days at the assigned hours. 10. Reports to and requests supervisor prior approval for any changes requested by the client to the schedule, hours or tasks. 11. Notified the office supervisor or On Call Supervisor after hours in advance (min of 8 hours) if unable to work as scheduled due to illness or emergency. (no shows to scheduled hours of work will be investigated by the supervisor and possible disciplinary actions will be taken. Two events of a NO show or call to the office supervisor will be grounds for immediate dismissal. 12. Responsible to utilize EVV and follow the procedures for their assigned client/clients by clocking in and out daily as instructed by their supervisor. Since the agency will be reimbursed only for the authorized hours, all employees will not be allowed to work more than authorized hours. Any hours worked above/more than authorized will not be paid. i understand i cannot be paid if i do not utilize EVV to record my hours worked as instructed. 13. Responsible to notifying supervisor of all on and off the job injuries and illness where antibiotics have been prescribed. 14. Responsible to notify supervisor of any patient injury or changes in condition as soon as becoming aware. 15. Responsible to report immediately to supervisor when client enters the hospital, nursing home or leaves home and there will be a break in service, or any other reason that the attendant is unable to complete assigned days or hours. 16. Responsible to immediately report to supervisor any communications and/ro observations that client makes regarding potential harm to self or to others. If anyone is in imminent danger, the attendant is responsible to notify the police. 17. Responsible to report immediately to the immediate supervisor or management for any suspected abuse, neglect and exploitation, or orientation will be provided prior hiring to cover all procedures of reporting.

Employees of adept home Health providing" companionship services" as defined by the FLSA (Fair Labor Standards Act) certified nurse aides, attendants, homemakers, and home health care aides will be paid according to the provisions of the Fair Labor Standards Act effective January 1, 2015.**
