Sevita Physical Therapist - Occupational Therapist in San Dimas, California

NeuroRestorative, a part of the Sevita family, provides rehabilitation services for people of all ages with brain, spinal cord and medically complex injuries, illnesses and other challenges. In a variety of locations and community-based settings, we offer a range of programs, including vocational and therapy programs, day treatments, and specialized services for infants, children, adolescents, Military Service Members and Veterans.

Occupational Therapist/Physical Therapist with Dual Licensure

  • San Dimas, CA

  • 15-bed facility; low patient ratios

  • CLHF (Congregate Living Health Facility)

In the Rehabilitation Occupational/Physical Therapist role, you will be an important part of our commitment to serving others by providing medically prescribed therapy treatment for patients suffering from brain injuries, spinal cord injuries, deconditioning, or muscle, nerve, joint, and bone diseases to restore function, relieve pain, and prevent disability.

  • Evaluate and test individuals served and develop effective treatment plans while providing quality service.

  • Administer treatment programs for individuals served and communicate treatment needs to staff.

  • Provide re-evaluation, programmatic changes, and effectively manage a caseload.

  • Set up appropriate home exercise programs for individuals served and perform home evaluations.

  • Maintain documentation including report of services, weekly schedule to meet therapy goals of staff and individuals served.

  • Serve on rehabilitation treatment team, including meetings, attending conferences, departmental meetings for program development, and establishing a system of operations for team enhancement.

  • Collaborate with physicians, staff, professionals/paraprofessionals, consultants, families, and community resources to ensure effectiveness of occupational therapy program.

  • Evaluate program participants through appropriate procedures and develop written treatment programs for each participant

  • Administer appropriate treatment programs and communicate program participant needs to appropriate staff; provide needed re-evaluation and programmatic changes as necessary

  • Maintain timely and appropriate program participant/administrative and department documentation

  • Ensure adherence to accreditation standards, licensure standards, professional code of ethics, and confidentiality

  • Attend programmatic meetings on request for team reports, policy setting, exchange of information, and other appropriate meetings/conferences as needed Attend clinical meetings to exchange, discuss, and evaluate program participant treatment programs

  • Serve on an interdisciplinary rehabilitation treatment team(s); communicate with physicians, staff, professionals/paraprofessionals, consultants, families, and community resources as needed to ensure the effectiveness of physical therapy program

  • Plan and implement in-service programs for staff and other groups as requested/assigned by supervisor

  • Participate in orientation programs for new staff, program participants, and PT/PTA and OT/OTA students

  • Write service summaries as requested by case manager/program director and maintain daily billing summaries and turn them into the appropriate staff

    Clinical Setting and Rehab Team

  • Treat patients in an inpatient setting

  • Develop treatment plans focused on community independence and functional outcomes.

  • Strong focus on cognitive rehabilitation

  • The rehab team consists of Physical Therapy, Occupational Therapy, and Speech Therapy and Respiratory Therapist, working alongside nursing services.

  • Therapy-led administration.

    Qualifications:

  • Bachelor’s Degree in Occupational Therapy and one-year human services experience

  • Bachelor's Degree in Physical Therapy (DPT preferred)

  • One year experience in Neuro-rehabilitation preferred

  • Must be licensed OT/L in CA and a member in good standing with American Occupational Therapy Association (AOTA)

  • CA state licensure in physical therapy

  • Experience in the field of neuro rehab and brain injury preferred

  • Advanced certifications highly preferred: Certified Neuro Specialist (CNS), Neurologic Certified Specialist (NCS), Neuro-developmental Treatment Certified (NDT), Neuro-IFRAH®, and/or Certified Brain Injury Specialist (CBIS).

  • Experience in physical agent modalities

  • Current valid CA driver's license , car registration, and auto insurance

  • For therapists traveling between/covering multiple facilities (if applicable)

  • In addition, when traveling from one facility to the next in the same day, mileage is paid out at the current federal rate; $0.67 per mile at time of posting
