Job Description:

We are seeking a passionate and experienced Licensed Community Social Worker to join our team as a contractor. As a Licensed Community Social Worker, you will play a crucial role in overseeing our first-ever community health worker based contract to deliver our Connect for Life program that is currently serving hundreds of thousands of members nationwide. Your role will be to coordinate and refer eligible members to our team of community health workers (CHWs) to ensure the effective delivery of CHW services to individuals and families in need. You will collaborate with various stakeholders, including our informatics team, Chief Medical Officer, and local CHWs on the ground, to develop, audit, and implement care plans tailored to meet the unique needs of the communities for this contract.

Responsibilities:

  • Leveraging data from health plan claims in partnership with our informatics team, assess the needs of individuals and families in the community and create and coordinate the appropriate CHW referrals to address said needs.

  • Maintain accurate and up-to-date records of member interactions, assessments, and service referrals.

  • Prepare reports and documentation required for program evaluation and billing purposes.

  • Ensure compliance with all legal and ethical standards related to client confidentiality and data privacy.

  • Ensure compliance with all legal and ethical standards related to the operationalization and billing of CHW services in the state of CA.

Requirements

  • Master's degree in Social Work or related field from an accredited institution.

    • Current license with NPI as a Licensed Clinical Social Worker (LCSW) in the state of California.

    • Strong experience working with diverse populations especially those enrolled in Medicaid.

    • Excellent communication, interpersonal, and leadership skills.

    • Ability to work collaboratively in a multidisciplinary team.

    • Familiarity and experience with the oversight requirements and claims submission process for CHW’s in the state of CA extremely desirable.
