Robert Half Case Manager in San Pedro, California

Description

We are seeking a compassionate and dedicated Homeless Housing Case Manager to join our client's team. The primary responsibility of this role is to provide case management services to homeless individuals, assisting them in securing and maintaining stable housing. The Case Manager will work closely with clients to assess their needs, develop individualized housing plans, and connect them with appropriate resources and support services.

Key Responsibilities:

  • Conduct comprehensive assessments of clients' housing needs, barriers, and strengths.

  • Develop and implement individualized housing plans in collaboration with clients, addressing their unique circumstances and goals.

  • Assist clients in navigating the housing application process, including completing paperwork, gathering necessary documentation, and advocating on their behalf.

  • Coordinate with landlords, property managers, and housing agencies to secure housing placements for clients.

  • Provide ongoing support and advocacy to clients throughout the housing search and placement process.

  • Connect clients with community resources and support services, such as employment assistance, healthcare, and financial counseling.

  • Maintain accurate and up-to-date case notes, records, and documentation in accordance with program guidelines and regulations.

  • Collaborate with interdisciplinary team members, including social workers, counselors, and outreach workers, to ensure holistic care and support for clients.

    Requirements

Qualifications:

  • Previous experience working with homeless populations or in housing services.

  • Knowledge of housing resources, rental assistance programs, and landlord-tenant laws.

  • Strong communication, interpersonal, and case management skills.

  • Ability to work independently and as part of a team in a fast-paced and dynamic environment.

  • Compassion, empathy, and a commitment to social justice and equity.

  • Proficiency in Microsoft Office and case management software (preferred).
