Description:
Sunnyview Rehabilitation Hospital in Schenectady, NY is seeking a Care Management Liaison .
Responsibilities include:
Information dissemination and education about waiver services
Assist with interviewing prospective participants to determine eligibility for Waiver Services
Track referrals and intakes and manage related databases
Assist with development of Community Resources and Provider networks
Assist with review of all participant service plans
Provide technical assistance to participants, family members and providers
Minimum Requirements:
Education:
Possess a BS/BA degree or higher in health or human services and 4 years of combined satisfactory profession experience working with individuals with disabilities or seniors
OR
Possess an Associates degree and 10 years of combined professional experience working with individual with disabilities or seniors
Skills/Experience:
Demonstrated expertise working with individuals with disabilities and seniors and extensive familiarity with the operation of 1915 (c) waivers and community-based services highly preferred
Knowledge, skills, and/or abilities to assess, identify and address gaps in services
Excellent screening and interviewing skills
Excellent communication and presentation skills
Ability to develop and maintain collaborative relationships with regionally based stakeholders, including LDSS, other local government entities, providers, advocacy organizations and others necessary to assure a comprehensive coordinated approach to services for individuals with individuals with disabilities and seniors
Working knowledge of community-based resources for individuals with disabilities and/or seniors
Excellent problem-solving and investigating skills
Working knowledge of Medicaid, Medicare and/or other third party payers
Skill in coordinating activities, evaluating data and establishing priorities
Pay Range: $17.85-$28.30 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
