Responsibilities Job Summary The Advanced Practice Nurse Practitioner Clinical Program Manager (APCPM) is responsible for the Advanced Practice Provider (APP) practice model in the assigned specialty clinics and affiliated clinical pathways and programs. This includes, but is not limited to, the oversight of the clinical nursing/provider outpatient practices and care rendered in inpatient hospitals. The APCPM collaborates with multiple professionals to support and coordinate the provision/management of patient care. This role will also work with the specialty Pillar Chief to develop and execute clinical and programmatic goals aligned with the overall specialty practice and the cardiovascular service line (CVSL) strategic objectives and initiatives. The APCPM is responsible for ensuring the delivery of high-quality and cost-effective care, based on competency and evidence-based clinical practice and research. The APCPM will work closely with physicians and administrative practice leaders to provide applicable clinical expertise, clinical leadership, and programmatic development for assigned cardiovascular specialty practice services within the HonorHealth Medical Group. The APCPM will work collaboratively with CVSL physicians and administrative leaders to bridge or align practice-based or new programs and services with the service line's hospital-based programs and services. This role works collaboratively with members of the healthcare team to provide care appropriate to the age of the patients served in their assigned practice. This position emphasizes health promotion and maintenance, disease prevention, and the diagnosis and management of acute and chronic diseases. Clinical and programmatic oversight of affiliated specialty Advanced Practice Providers (APPs) and clinical support staff in collaboration with organizational leaders will enable a standardized and skilled approach to building evidence-based programs. In addition to providing both direct patient care and supervision of the APPs, the APCPM will manage appropriate coverage, evaluations, education, and training. The APCPM will assist in assessing affiliated staffing needs, and participate in recruitment, onboarding, and retention within the practice. The APCPM adheres to and functions within the scope of practice defined by the Arizona Board of Nursing and the organization. Adherence to evidence-based advanced nursing practices and utilizes a Shared Governance model for decision-making. The practice focuses on performance standards that act in accordance with the organization's initiatives. This individual possesses an ability to work with all levels of staff within a multidisciplinary group, with a significant level of administrative and clinical autonomy, the APCPM is highly motivated and able to work independently. Serving as a clinician, supervisor, and program leader, functions as a mentor, educator, and researcher. As a national and regional influencer, makes professional contributions in their field of expertise including organization memberships, participation in committees and councils, completing publications, doing presentations, and conducting research. Clinical Care: Practice focused on Patient/Family: direct care practice using a holistic perspective; therapeutic partnerships with patients; expert clinical thinking and skillful performance; reflective practice; evidence-based guided practice; diverse health and illness management approaches. As a provider, the APCPM performs history and physical exams, order and interpret diagnostic studies, prescribe medications, consult ancillary departments and other specialists, and provide a smooth transition between various levels of care, including discharge and transition care pathways; collaborating with the interdisciplinary team including specialty physician, pharmacy, other outpatient services. Core responsibilities minimally include: • Diagnose and treat acute, episodic, or chronic illnesses. • Prescribe all necessary medications and treatments. • Perform comprehensive physical examinations of patients. • Develop and implement patient care management plans. • Facilitate referrals to other healthcare professionals and medical facilities. • Communicate with collaborating physician or specialist regarding patient care role and responsibilities. Evidence-Based Program/Services Development: The APCPM will function independently to direct the planning, development, and implementation of practice-based specialty programs and services. Works collaboratively for planning, development, and implementation of the cardiovascular center of excellence (CVCoE) and CVSL programs and services. The APCPM will work closely with service line physicians and administrative leaders to lead, advocate and support program innovation. Works to coordinate patient care delivery. Maintain environmental safety and orderly patient flow. Coordinates activities and interacts effectively with other departments and medical staff to manage the patient care environment. Will organize, lead, and collaborate in the development, implementation, and maintenance of standardized, multi-disciplinary care processes, protocols, and tools based on current evidence-based practice and/or established clinical guidelines. Identify gaps in care continuum nursing/provider knowledge and works collaboratively with nurse educators/navigators/coordinators to deliver education opportunities that safeguard the use of best clinical practice and up-to-date guidelines and care standards. Collaboratively develops goals and expectations related to the program, clinical care, and quality performance. Leads, advocates, and/or supports the development of key systems to provide the best patient experience and access points with efficient patient care navigation. Supports CVSL Pillar Chiefs, Medical Directors, and Quality Consultants in quality and registry process improvement including review, planning, and initiative selection required to drive better clinical outcomes. Education and Oversite: Leads and/or supports the practice and CVSL clinical leadership in the development of interdisciplinary and nursing/provider standards relevant to scope of practice. Ensures efficient and effective delivery of practice by nonphysician providers and staff clinical education content. Encourages and facilitates professional growth and development of APPs and staff. Offers clinical input to staffing needs to address identified gaps in care. Advisor to ensure adequate staffing levels, inclusive of hiring, evaluating, managing performance, coaching, and developing employees. Provides a practice environment that encourages a team culture responsive to the provision of patient care services emphasizing employee retention and productivity. The APCPM will continue to monitor new/updated expert consensus recommendations to ensure that assigned specialty practice is delivering precision care Marketing, Program Growth and Professional Contribution: Leads the development of marketing and education outreach initiatives and material for referring physicians/providers, patients, and the community to improve growth, awareness, access, and care navigation. Provides mentorship and guides professional growth and training to newly graduated/hired providers. Role models and encourages the professional growth of APPs including involvement in professional organizations. As a national and regional influencer, makes professional contributions in the CNP’s field of expertise including memberships in professional organizations and participation/leadership in committees and councils. Active in doing organizational and podium presentations, completing publications, or conducting research.
