(Tillamook, Clatsop, Columbia counties) Job Summary This position is responsible for managing the Regional Care Teams within a CCO. This position provides direct oversight for all care management activities, integrated clinical health services provided through the Regional Care Teams and the implementation of other Population Health Program (PHP) initiatives. This position functions as a member of the CCO leadership team and the PHP Management/Leadership Team, participating in strategic planning and design of clinical operations, managing the work of the RCT, and evaluating outcomes to ensure high quality member centered support and continuous process improvement. Essential Responsibilities Provide clinical and operational leadership for staff providing care management services and other clinical operations and initiatives through the Regional Care Teams. Responsible for the oversight and clinical supervision of Regional Care Team staff, including a Care Team Supervisor, clinical care coordinators, and non-clinical care coordinators. Provide clinical leadership in the development and review of clinical protocols and processes that support member centered care plans, appropriate referrals, and consistent quality care for members. Partner with the PHP Management/Leadership Team and the CCO or regional leadership team to promote strong teamwork, collaboration, and integration. Plan, coordinate, and oversee delivery of clinical services in compliance with all State and Federal regulations. Work closely with other managers across the organization to ensure integration, collaboration, and support of organizational activities and goals including the development, management and evaluation of care enhancing cost-control programs and population health initiatives in collaboration with delivery system partners, CCO or regional leaders, and social service agencies. Participate in the implementation of strategic clinical initiatives for members that reduce cost and improve health and quality of life while meeting the CMS SNP model of care requirements. Identify gaps in care for our membership and work collaboratively to remove these barriers. Actively participate in annual program goal setting that aligns with organizational strategic goals. Responsible for participating in the planning and implementation of strategies related to quality standards and metrics. Support clinical collaboration with delivery system leaders in the region to build a strong foundation of member centered care and population health program support for statewide primary, secondary and tertiary delivery systems. Develop and maintain strategic and operational partnerships with community-based social service organizations to assure we can leverage and improve access to upstream non-medical health-enhancing services for its members especially for those experiencing a transition in care.
