Bellefaire JCB Foster Care Case Management Aide in Shaker Heights, Ohio

Position Summary:

The Foster Care Case Management Aide will be involved in clinical and administrative work and direct contact with foster children and foster parents. Direct contact will focus on clinical issues in the treatment plan as well as the role of a support system. The aide will consult with the rest of the treatment team regarding any issue that may arise concerning children and/or foster parents.

Responsibilities Include:

  • Visit foster children and foster parents in the foster home to monitor progress, assess for special needs, and ensure implementation of treatment plans.

  • Provide support and supervision of foster parents during home visits and in weekly cluster meetings.

  • Attend weekly foster parent cluster meetings to assist case manager with facilitating the group or with supervision of children.

  • Meet with foster children during monthly cluster meetings as needed.

  • Assist in the training of foster parents as needed.

  • Provide transportation as needed for foster children to visits and appointments etc.

  • Maintain assigned client records in compliance with ODJFS, and other accrediting organizations, licensing regulations; including documentation of client contacts, writing and revising individual treatment plans, ensuring that medical and dental appointments are completed at appropriate intervals and documented in the record.

  • Assist Case Manager in assessing and evaluating foster families.

  • Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.

  • Respect the privacy of clients and hold in confidence all information obtained during the client’s treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material.

Qualifications:

  • Minimum Bachelor’s Degree

  • Experience working with children, adolescents and their families
