Purpose of this Position:

The primary objective of this role is to build and maintain relationships with community organizations through volunteer efforts, in-person meetings, and by spearheading a collaborative outreach initiative. In this position, you'll actively engage in community outreach by going door-to-door and connecting with community partners, service providers, housing agencies, and the broader community. This position serves as a co-facilitator for a joint initiative with the public health department aimed at enhancing communication between providers and outreach workers. The primary objective is to create and maintain positive relationships between community organizations and services to promote referrals, boost communication and collaboration, as well as to aid in creating a wider "net" of wrap around services for our community.
