Medication | | | Administration:Administer | | | prescribed medications to | | | patients accurately and | | | safely, following established | | | protocols and procedures. | | | This includes oral | | | medications, topical | | | medications, injections, and | | | other forms of medication as | | | prescribed. | | | | | | 2. Medication | | | Monitoring:Monitor patients | | | for anyadverse reactions or | | | side effects following | | | medication administration. | | | Report any concerns or | | | changes in patients' | | | conditions to the nursing | | | staff promptly. | | | | | | 3. Medication | | | Reconciliation:Assist in | | | medication reconciliation | | | processes by obtaining | | | medication histories, | | | reviewing medication orders, | | | and updating medication | | | records as necessary. | | | | | | 4. Documentation:Maintain | | | accurate and up-to-date | | | medication records, including | | | documentation of medications | | | administered, doses given, | | | and any relevant observations | | | or assessments. | | | | | | 5. Patient Education:Provide | | | medication education to | | | patients and their families, | | | including instructions on | | | medication administration, | | | potential side effects, and | | | precautions to take. Answer | | | any questions or concerns | | | patients may have regarding | | | their medications. | | | | | | 6. Assistance with Activities | | | of Daily Living | | | (ADLs):Assist patients with | | | activities of daily living, | | | such as bathing, dressing, | | | grooming, and toileting, as | | | needed. Provide support and | | | encouragement to promote | | | patients' independence and | | | well-being. | | | | | | 7. Compliance and | | | Safety:Adhere to medication | | | administration policies, | | | procedures, and regulatory | | | requirements to ensure | | | compliance with state and | | | federal laws. Maintain a safe | | | and clean medication | | | administration environment, | | | following infection control | | | protocols and medication | | | storage guidelines. | | | | | | 8. Collaboration:Work | | | collaboratively with other | | | healthcare team members, | | | including nurses, physicians, | | | and other allied health | | | professionals, to ensure | | | coordinated and comprehensive | | | patient care. | | | | | | Qualifications: | | | | | | 1.
