UPMC Outpatient Nurse Coordinator - Somerset Family Practice in Somerset, Pennsylvania

Responsibilities:

  • Take responsibility for your professional development to enhance practice quality and patient care.

  • Engage in shared governance, goal setting, and support changes to improve care and the practice environment.

  • Be an active partner on the care team, assisting and collaborating with team members.

  • Adapt to changes and demonstrate flexibility in the process.

  • Oversee and supervise the practice environment, assisting other disciplines as needed.

  • Lead and mentor new nursing staff, patient information coordinators, and office assistants.

  • Develop patient care plans that involve patients as partners and consider their holistic needs.

  • Assist physicians with patient care, including assessment, evaluation, and education.

  • Maintain productive relationships with colleagues, demonstrating accountability, enthusiasm, and commitment.

  • Participate in quality improvement efforts and take responsibility for enhancing patient satisfaction.

  • Use critical thinking to identify clinical, social, safety, psychological, and spiritual issues in patient care.

  • Communicate effectively, translating patient conditions to other care providers and recommending changes.

  • Ensure comprehensive patient documentation to promote caregiver communication.

  • Align with national and organizational goals to improve patient safety, quality, and satisfaction.

  • Build healing relationships with patients, families, and colleagues to create a compassionate experience.

  • Apply adult learning principles in teaching patients, families, students, and new staff.

  • Provide detailed guidance to patients and families throughout their care, both in-office and at home.

  • Act as the primary nursing liaison for developing total care plans for patients and families.

  • Support the development of students, new staff, and colleagues, possibly serving as a preceptor.

  • Foster an environment of open dialogue and continuous development by seeking feedback and improving practices.

  • Minimum 3 years nursing experience

  • BSN preferred.

Licensure, Certifications, and Clearances:

  • Registered Nurse (RN)

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • UPMC approved national certification preferred.

  • Act 34

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
