State of Massachusetts Services Coordinator Generalist in SouthYarmouth, Massachusetts
The Department of Developmental Services (DDS) is seeking a motivated and experienced human service professional for the role of a Services Coordinator Generalist (Human Services Coordinator I)in the Cape and Islands Area Office located in South Yarmouth. The Services Coordinator Generalist monitors the safety and wellbeing of assigned individuals and advocates in regard to their human, civil, and legal rights through the implementation of support services, the Individual's service plan, and other necessary means. _Duties and Responsibilities:_* (these duties are a general summary and not all inclusive) * Monitor DDS funded and auxiliary funded programs. * Maintain regular contact with individuals, families, and providers. * Travel to individuals' homes, programs, and outside meetings; that may or may not be in close proximity to the Area Office. * Coordination, facilitation, and monitoring the development and progress of support plans per DDS regulations. * Utilize the communication methods needed for assigned caseloads. * Provide outreach and coordinate referrals to community services and other state agencies. * Provide up-to-date record keeping – both electronic and hard copy; maintain documentation for waiver or non-waiver services. *_Preferred Qualifications_: * Familiarity with Microsoft programs and Home and Community Services Information System (HICSIS). * Experience working with individuals with Intellectual Disabilities and/or who are Deaf and Hard of Hearing. * Ability to develop and maintain respectful, caring, patient, ethical, and empowering relationships with families and individuals supported by the department. * Excellent organizational and communication skills * Ability to utilize American Sign Language and/or bi-lingual skills * Skills using communication styles and/or augmentative communication systems _Schedule Information: _ Full-time – 37.5 hour weekly position.
