Your Responsibilities as a Hospice Social Worker

  • Comprehensive Patient Assessment: Evaluate patients’ psychosocial, financial, and environmental needs to create a holistic plan of care.

  • Individualized Care Planning: Develop and adjust care plans that support patients and families through each stage of hospice, ensuring personalized support.

  • Emotional Counseling & Support: Provide compassionate counseling and guidance to patients and families, helping them with challenging decisions and end-of-life planning.

  • Advocacy & Coordination: Serve as a patient advocate, ensuring they receive necessary resources and support, and work closely with doctors, nurses, and team members for coordinated care.

  • Safety Monitoring & Reporting: Keep patient environments safe, report concerns, and implement safety measures when needed.

Your Qualifications

  • Education: Master’s or Doctoral Degree in Social Work from an accredited program.

  • Experience: At least one year in a healthcare setting preferred, with a focus on end-of-life care.

  • Licensing: Current state license as a Licensed Social Worker (LSW).

  • Travel: Ability to travel to patient homes within Springfield to Jacksonville; valid driver’s license and car insurance required.

  • Skills: Strong communication skills with an understanding of DNR protocols and end-of-life care (preferred).
