ABOUT PATHLIGHT: Pathlight (formerly known as The Association For Community Living) has been supporting and empowering individuals with developmental and intellectual disabilities (and their families) in Western Massachusetts since 1952. Title: Manager 2 (Social Worker) / Family Services, Family Based Living Location: Springfield and Pittsfield - Manager will supervise a caseload split between two locations. 220 Brookdale Drive, Springfield, MA 01104 75 S Church Street, Pittsfield, MA 01201 Summary of Position: Responsible for the overall case management of individuals with developmental disabilities placed in Adult Family Care, Shared Living homes, or Intensive Therapeutic Foster Care for children. Coordinate/monitor delivery of services to those supported by Care Providers, Foster Parents, Primary Support Staff, Individual Support Specialists and Respite Providers to ensure conformity to funding agencies regulations, internal policies, proc edures and quality standards, and to applicable program model. Identify stressors and conflicts and strategize appropriate interventions to maintain stability. Provide supervision to Individual Support Staff (ISS) and Personal Support Staff (PSS) employees. Work collaboratively with the Department of Developmental Services, Department of Children and Families, MassHealth, and other state and/or private agencies. Prepare all required reports assessments as they relate to the caseload in a concise, comprehensive and timely manner. Provide crisis intervention services as needed. Provide scheduled emergency on call support as part of the on-call system. Knowledge and Experience Required: Broad knowledge of a discipline such as, human services or social work and completion of a 4 year college education. Over 3 years and up to and including 5 years of progressive learning experience. Working knowledge of computer systems, programs, and applications. LSWA or eligible. Valid driver\'s license, reliable transportation/vehicle, as well as the ability to travel. Reporting Relationships: Reports to designated supervisor. Responsible for supervising direct support staff and care/respite providers. Hours: 40 hours per week, Monday - Friday, 9:00 am to 5:00 pm with flexibility for Program needs. Plus on-call rotation. SALARY: \$48,380.80 - \$52,353.60 / year starting range. This position is eligible for hiring to the discretionary Hire Point depending on experience.
