Serenity Hospice Care Hospice Bereavement Coordinator in Springfield, New Jersey

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences.

Serenity Hospice Care is looking for an enthusiastic and organized Bereavement Coordinator for our Springfield, NJ office. This position is full time, Monday through Friday from 8:30am to 4:30pm.

Responsibilities:

  • Make all bereavement calls throughout the 13 months of bereavement.

  • Make Moderate (6 months and 12 months) and High Risk (monthly) calls on schedule.

  • Plan, implement, maintain services necessary to patients, identified caregivers, hospice staff, and the community at large.

  • Provide updates to the bereavement plan of care as necessary.

  • Attend Interdisciplinary Team meetings and participates in the development of the patient/family/caregiver bereavement and comprehensive plan of care.

  • Coordinate services with the Interdisciplinary Team regarding family/caregiver bereavement needs.

  • Plan for Bereavement Program development, expansion and refinement through annual evaluation of services.

  • Provide bereavement support to hospice personnel coping with work related grief through one-to-one counseling and referral to community resources.

  • Conduct specialized bereavement training and provides oversight for volunteers providing bereavement services.

  • Keep up to date with bereavement and grief counseling trends and knowledge.

  • Performs other duties as assigned.

    Qualifications:

  • Must have at least two years of Hospice experience.

  • Excellent computer skills, especially Microsoft Office.

  • Flexible and able to adapt to changes as needed.

  • Able to self-manage and work with teams

  • Organized & Detail Orientated.

  • Reliable and trustworthy.

  • Drivers License with reliable transportation.

This position is full time, Monday through Friday from 8:30am to 4:30pm.
