Northeastern Vermont Regional Hospital Community Health Program Manager in St. Johnsbury, Vermont

Northeastern Vermont Regional Hospital (NVRH) seeks a Community Health Program Manager to join the team! As the Community Health Program Manager, you will oversee the implementation of Vermont's Blueprint for Health model in the hospital service area, serving as the primary local contact responsible for managing programmatic and administrative components of implementation. The Community Health Program Manager will work collaboratively to facilitate alignment with the departmental, OneCareVT Accountable Care Organization (ACO), and Blueprint objectives within the NVRH health service area.

Essential Responsibilities:

Healthcare Integration: Collaborate with internal and external partners to improve community health and pursue additional funding opportunities through grants.

Collaboration and Quality Improvement: Partner with quality teams to enhance care coordination, clinical quality, and patient outcomes.

Community Engagement: Lead outreach and collaboration efforts, including participation in community health assessments and NEK Prosper initiatives.

Grants Management: Oversee administrative and financial functions for Blueprint and related grants.

Care Coordination: Support primary care practices and promote healthcare integration by troubleshooting barriers and facilitating effective care coordination.

Qualified candidates should demonstrate a comprehensive knowledge of care coordination models and methods, including financial payment models for primary care and continuum of care providers and process and system improvement models. To be successful in this role, candidates should have significant experience in a healthcare environment, community engagement skills, and experience utilizing data-informed approaches to achieve desired population health objectives.
