Actalent Hospice RN Care Manager in St. Louis, Missouri

Seeking a dedicated Remote Case Manager to join our team, focusing on reaching out to Medicare members to enroll them in the Care Management program. The successful candidate will build care plans, conduct medication reviews, and assess care plans to ensure the best outcomes for members. This role requires completing approximately 20 calls per day, evaluating member needs, and developing ongoing care plans. Collaboration with providers to identify specialists or community resources is crucial to address unmet needs of the members. The role also includes identifying barriers to care, coordinating between members and their care providers, and providing ongoing follow-up on member status changes. Additionally, the manager will offer resource support in areas like employment and housing and educate members and their families on disease processes, care options, and healthcare benefits.

Needed:

  • RN Compact License

  • Experience with Medicare

  • Home healthcare/Hospice experience

  • Remote Case Management experience

  • Utilization Management software (Trucare preferred)

Soft skills:

  • Ability to coordinate and facilitate care plans

  • Effective communication with members, families, and healthcare teams

  • Problem-solving and barrier identification

  • Providing educational support and resources

Contract Role
