BKJ Peoples Heal Centers is seeking a Hospital Community Liaison to join our team .This position supports the Hospital Liaison Linkages Program by working in collaboration with case management and social work staff at area hospitals. The expectation for this position is that the Liaison will travel each week to area hospitals to develop relationships with discharge planners and case managers, review files of admitted adults and make appropriate referrals. PRIMARY RESPSONSIBILITIES AND DUTIES: Maintain confidentiality of records relating to clients' treatment. Acts as the first point of contact (liaison) for the hospital staff when a referral is made for OP mental health services. Develop specific hours to travel to local hospitals assigned and develop relationships with discharge planners and seek referrals. Provides on-site preliminary screening and initial evaluations of mentally and emotionally disturbed clients who reside in the catchment area. Collaborates with referral staff and relevant family to gather all relevant clinical, financial, social and psychological information required for admissions review. Act as client advocates in order to coordinate required services or to resolve emergency problems in crisis situations; conducts follow up assessments to all clients calling the crisis line or who are in need of walk-in crisis intervention. MINIMUM EDUCATION, TRAINING AND EXPERIENCE: Requires a Master's degree from an accredited school in Social Work, Psychology or Nursing. Requires working knowledge of the DSM-IV and diagnostic terminology. Requires the ability to develop and implement individual treatment plans with flexibility and independence, to utilize case work methods, supervision and consultation in practice; to function within prescribed policies and procedures of the Center; to communicate professional content orally and in writing; to maintain effective relationships with other staff disciplines and community agencies. Candidate must have a minimum of two (2) years post graduate clinical experience in a variety of treatment modalities. Must have good computer skills, be able to multi-task and demonstrate strong organizational skills.
