Description of Job

The Work Unit:

Division of Disease Control and Public Health Response (DCPHR)

DCPHR’sHealth Equity Branch exists to expand and promote equitable access to opportunities and resources that impact an individual’s or community’s health and right to be free from communicable diseases and their health effects. The Branch partners with community-based organizations, healthcare entities and local public health agencies in Colorado by promoting health equity and implementing strategies tailored to address the varying complex causes of health disparities that often stem from structural racism or the historical disenfranchisement and discrimination of marginalized groups, including racial and ethnic minorities, low-income populations, and members of the LGBTQ community. The Branch works collaboratively to set strategic priorities, collect and disseminate data, identify and implement effective strategies, and align resources within the division and across the department and other state agencies.

The Health Equity Branch includes aResource Support Team Unit. This Unit consists of several lines of work focused around establishing and maintaining 10 regional support networks for community based organizations, healthcare entities, and local public health agencies in Colorado, primarily addressing the impacts of COVID-19 on community health. The regional support networks are spread geographically throughout the State, to better help and support the unique needs of community-based groups focused in different regions across the state in accessing funding and other resources to support programs that aim to advance health equity and address social determinants of health as they relate to COVID-19 health disparities among populations at higher risk and that are underserved. The Resource Support Team Unit consists of 10 resource specialists (each with a specific geographic region of focus), 2 team leads, 1 program evaluator, and a unit manager. The Resource Support Team Unit is responsible for assisting community-based organizations and other entities in accessing funds/resources through CDPHE for programs related to COVID-19 around cultural/community health navigation, wrap-around resource support, and community engagement and communications. The Regional resource specialists, under direction of the team leads and unit manager, are responsible for directing the overall portfolio of funded work, including program/contract monitoring, within each region and across the state to assure community needs are met in region populations experiencing a disproportionate burden of COVID-19 infection, severe illness, and death to guide the response to the COVID-19 pandemic.

DCPHR, and the Health Equity Branch, operate on the belief that all people - no matter their race, ethnicity, gender identity, sexual orientation, education level, age, language, religion, ability or geographic location – should have the opportunity to live healthy lives.

What You Will Be Doing:

The 10 Regional Equity Resource Specialists are each responsible for assisting community-based organizations and other entities in their assigned region in accessing COVID-19 related funds/resources through CDPHE. The Equity Resource Specialist is responsible for being knowledgeable in region-specific needs and ensuring the overall portfolio of funded work within their assigned region comprehensively covers community needs.

Responsibilities of the position include but not limited to:

Subject Matter Expertise - Funding

  • Serves as a subject matter expert for work in the Resource Support Team Unit’s Regional Resource/Funding Support network.

  • Responsible for understanding their region’s specific community needs and the variety of funding opportunities/funding mechanisms available to community based organizations and entities through CDPHE across various programs in their assigned region.

  • Assists community groups in their assigned region in identifying the correct funding mechanisms and resources for their activities and programs. Is aware of relevant funding opportunities outside of CDPHE, to direct organizations towards alternative options when CDPHE funding is not an appropriate fit.

  • Provides subject matter expertise related to allowable expenditures and programmatic content of various funding mechanisms, including an understanding of both state and federal limitations on allowable spending.

  • Maintains awareness of grant program requirements, goals, and objectives, while ensuring compliance with funding requirements and laws.

Collaboration/Coordination

  • Understands the complete picture of available resources and goals of community needs in assigned region to assess whether the portfolio of funded groups and projects is fulfilling needs of the community.

  • Coordinates with other regions to ensure cohesive support for community based organizations related to identifying funding to meet community needs and implementing work with that funding to serve community needs.

  • Other duties as assigned.

Minimum Qualifications:

(OPTION 1) Experience Substitution

Six (6) years of professional experience in the following, or a combination of,: grant administration, contract management, coordinating grants, securing funding, working with diverse stakeholders and community members with a wide range of backgrounds and identities in pursuit of common goals, neighborhood organizations, knowledge of the community organizations, programs and social issues regarding residents of Colorado.

OR

(OPTION 2)

Two (2) years of professional experience in the following, or a combination of,: grant administration, contract management, coordinating grants, securing funding, working with diverse stakeholders and community members with a wide range of backgrounds and identities in pursuit of common goals, neighborhood organizations, knowledge of the community organizations, programs and social issues regarding residents of Colorado AND a b achelor's degree insocial science, human services, public health, environmental health, community health, public administration or closely related field of study related to the work assignment.

OR

(OPTION 3) Advanced Education

A Master's degree in S ocial Science, Human Services, P ublic Health, Environmental Health, Community Health, Public Administration, or a field of study closely related to the work assignment.

  • Relevant experience with the State of Colorado.

  • A passion for social justice and for the mission of DCPHR

  • Experience with a variety of funding opportunities available to community

based organizations.

  • Experience with grant funding.

  • Previous experience with collaborative efforts at the state, local, or community

level.

  • Previous experience monitoring contracts, deliverables, and budgets.

  • Previous experience in disproportionately impacted communities.

  • Experience collaborating with stakeholders and partners, including providing

technical assistance.

  • Excellent communication, organizational, critical thinking, interpersonal and

problem solving skills.
