SUNY Upstate Medical University Nurse Practitioner / Physician Assistant in Syracuse, New York
Job Summary:
Highly motivated Nurse Practitioner/Physician Assistant to work as part of both the Movement and Cognitive Disorder teams to provide direct patient care in an outpatient Neurology setting.
The NP/PA will assess, diagnose and develop a plan of care in accordance with practice protocols in the Neurology out-patient ambulatory care setting. Responsibilities in the Movement and Cognitive Disorders Clinics include:
Movement Disorders:
Basic management and programming of DBS patients with essential tremor and Parkinsons disease
Function as a liaison between movement disorders program and functional neurosurgery program to facilitate prompt evaluation and monitoring of the presurgical evaluation and postsurgical return to the movement disorders program for programmin
Evaluation and management of follow ups and some new patient evaluations for tremors and other movement disorders when deemed appropriately by neurologist.
Cognitive Disorders:
Evaluation and management of patients with cognitive disorders including Alzheimers disease, Lewy body dementia, vascular cognitive impairment, frontotemporal dementia, and autoimmune dementia.
Works collaboratively with the attending physician to identify, develop, and implement an appropriate plan of care that maximizes individual patient/family preference and enhances quality, access, and cost-effective outcomes.
Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care.
Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or with the collaborating physician.
Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol.
Facilitates the continuity of care across the continuum of services, e.g. home care, other facilities, appointments with MDs etc.
Works collaboratively with nursing leadership to ensure timely communications with the nursing staff.
Minimum Qualifications:
NYS licensed/registered NP or PA required (NYS NP specialty area of certification must be consistent with practice). Must have NPI number and be Medicare Enrolled with Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within 6 months of hire (eligibility criteria for NP requires National Certification through AANP or ANCC, and Master's Degree) (eligibility criteria for PA requires graduation from an accredited PA program and National Certification through NCCPA). Initial certification must be present or obtained within 6 months of hire. Current BLS certification through the American Heart Association or the American Red Cross is required.
