SUNY Upstate Medical University Pharmacist - Transitions of Care in Syracuse, New York

Job Summary:

This pharmacist will primarily provide dedicated patient care, obtain medication histories, and have oversight for safe and efficient transitions of care from hospital admission to hospital discharge. They will work with a team of healthcare professionals to enhance the process of hospital admissions to other care settings. This includes integration with the pre-admission testing department and emergency medicine among other departments. Pharmacist coordination with both internal and external providers and nursing staff will be required.

In addition, this pharmacist will interpret medication orders and prescriptions to ensure that they are complete, safe, rational, and compatible with institutional policy. This includes preparation of compounds, including injectable products, and dispensing of medications for inpatient and ambulatory care use.

Performs duties in all areas of Pharmacy to include inpatient, ambulatory care and satellites. Provides information to health care practitioners to promote the rational use of medications. Provides drug monitoring service. Maintains Pharmacy records by law and institutional policy. Interprets policy and procedures for hospital staff. Participates in teaching programs for hospital staff. Participates in Pharmacy Departments Quality Assurance Programs.

Minimum Qualifications:

B.S. in Pharmacy or Pharm.D., NYS license and registration - or eligible - required; completion of PGY1 Pharmacy residency or equivalent experience required.

Preferred Qualifications:

Previous hospital experience preferred. Specific experience with obtaining medication histories and transitions of care activities preferred.

Work Days:

Rotating Shifts: Days, Evenings, Nights, Weekends & Holidays as needed. Operational staffing component.
