SUNY Upstate Medical University Registered Nurse - Transplant Coordinator in Syracuse, New York

Job Summary: Assesses, plans, implements, and evaluates the patient centered Transplant Services care provisions at the Upstate University Hospital Transplant Services Department. Collaborates with Transplant Services Administration, Transplant RN Coordinators and clinical team members, Social Work, Pharmacy, Dietary, and providers in the areas of living donor and transplant recipient care. Responsibilities include patient status evaluation, direct patient care provisions through all phases of living donor and transplant recipient direct patient care (pre, intra, and post operative), program development, implementation of policy and procedures, and participation, development, and review of clinical quality. Participates in the development of education curricula and materials by writing original material and/or reviewing the work of others. Participates in case management, telephonic triage, and patient care

planning and goal setting. Provides education services and general Transplant self-management education to patient and families, care partners and the community.

Minimum Qualifications:

Possession of or eligibility for a license to practice as a registered nurse in New York State and two years of post-licensure clinical nursing experience in an acute care specialty area.

Preferred Qualifications:

Previous experience with transplant.

Work Days:

Full-time (40 hours per week), Monday-Friday 730-1600

Salary Range/Pay Rate:

SG20- Based on experience + differential
