Job Summary and Qualifications

Organizes and facilitates activities associated with Performance Improvement for the hospital’s Sepsis/Stroke programs. Interacts with members of the Medical Staff, Nursing Department and other hospital department leaders and staff to ensure organization-wide understanding of program requirements. Employs techniques and tools associated with continuous Performance Improvement to assist teams in achieving goals for improvement. Works extensively with physicians, pharmacists, nurses, and other hospital staff in achieving support for the practice of evidence-based medicine related to clinical care of sepsis and stroke patients.

The Sepsis/Stroke Coordinator position is responsible for the data abstraction, aggregation and analysis of population data using Comet systems for input of data, analysis, comparison, and trending. The Coordinator provides educational support organization-wide as necessary regarding implementation of evidence-based initiatives, interpretation of the associated practice requirements, and data analysis. The Coordinator performs clinical concurrent review, conducts team meetings, and coordinates internal reporting. The Coordinator maintains an in-depth knowledge of hospital systems, patient clinical data, Meditech capabilities and reports, COMET capabilities and reports and external data bases used for public reporting and comparison. The Coordinator ensures the integrity of the data, and adheres to abstraction rules of CMS, TJC, HCA Outcomes Measurement Department, and Ethics and Compliance policies. The Coordinator develops systems for evaluating outcomes for clinical departments and physician compliance. The Coordinator facilitates Sepsis and Stroke-related Teams/task forces/Committees and initiates and coordinates an in-depth analysis on near or actual misses in compliance in collaboration with risk and quality staff, and develops strategies to achieve the desired results of measure sets.

Facilitates the development, implementation, and ongoing adherence of sepsis and stroke program protocols and therapies within the facility.

Position tasks and responsibilities include, but are not limited to:

  • Collaborates with medical and hospital staff to provide multidisciplinary care to patients with a diagnosis of Sepsis or Stroke,

  • Provides clinical expertise to care team, patients, and families,

  • Collaborates with physicians and staff in an effort to decrease mortality and length of stay,

  • Collaborates with the nursing team to ensure education is provided for patients and/or families with Sepsis or Stroke diagnosis,

  • Ensures the protocols and therapies become the standard of care followed within the facility,

  • Collects and analyzes Sepsis/Stroke Program data to assist with documentation, clinical care, and improved outcomes,

  • Coordinates Stroke/Sepsis meetings and participates in facility PI/Quality meetings as indicated,

  • Attend facility department meetings as needed for education purposes,

  • Collaborates with Education Coordinator for annual hospital-wide staff education plan related to Stroke/Sepsis education,

  • Maintains records as indicated for program reviews,

  • Conducts concurrent chart review of both Sepsis and Stroke patients and reports data as required/requested,

  • Performs data entry in Get with The Guidelines and The Joint Commission sites for Stroke Program,

  • Develops and provides education/outreach to EMS as requested,

  • Seeks out opportunities to provide community Stroke/Sepsis education,

  • Maintains knowledge of current Joint Commission disease specific guidelines and “Get with the Guidelines” evaluation criteria,

  • Participates in state-wide Stroke Improvement Committee,

  • Maintains accuracy in data entry and reporting evidenced by IRR,

  • Prepares graphs and reports to showcase data,

  • Timely and accurately measures the stroke criteria for The Joint Commission,

  • Coordinates evidence-based Sepsis program,

  • Analyses HCA and COMET sepsis data,

  • Participates in sepsis quality calls/meetings,

  • Recommends process improvement through evidence-based interventions with physician/staff input,

  • Develops, coordinates, and provides education to physicians, staff, and community on sepsis/stroke awareness and treatment,

  • Participated in stroke quality calls/meetings, and

  • Performs other duties as assigned.

What qualifications you will need:

  • Current Florida RN License

  • Graduate of an accredited nursing program, BSN preferred

  • Three (3) years experience in acute hospital setting preferred

  • Excellent communication skills

  • Ability to work with little or no supervision

  • Experience in Performance Improvement and Clinical Data Abstraction is required

  • Working knowledge of quality improvement/hospital regulatory standards preferred

  • Working knowledge of computer systems and programs. Advanced knowledge of Microsoft Office programs preferred
