Summary James A Haley Veterans Affairs is looking for Senior Social Worker (Emergency Department and Acute Care), who will report to Social Work Service, and is assigned to the Continuity of Care Program. The JAHVAH is a Level I tertiary medical center with general medicine, surgical, psychiatric, rehabilitation, spinal cord injury, nursing home care beds and one of five Polytrauma treatment facilities in the VA enterprise. The Senior Social Worker's scope is broad, and they provide care access across the hospital's care continuum even with limited supervision. The Senior Social Worker's practice areas include the Emergency Department, Community-Based Outpatient Clinics (CBOCs), Polytrauma Rehabilitation Center, Spinal Cord Injury Rehabilitation Center, and other satellite outpatient clinics during regular and off-tour hours. The Senior Social Worker ensures clinical program effectiveness consistent with the VA vision and mission. Additional duties include but not limited to: Is a diversity, equality, and inclusion champion that actively engages in strategic and tactical planning. Provides social work access during regular or off tour hours to meet the standards of national emergency department directive. A qualified supervisor or equivalent in the state of licensure, that can teach, coach, train, and mentor others in the profession of social work. Facilitates safe and appropriate disposition and discharge planning utilizing patients' available resources and benefits. Utilizes emergency department and acute care specialty areas outcome evaluations to improve treatment services and to design system change. Assessment and linkage skills in a range of specialized interventions and treatment modalities for supportive care services post hospital visit. Complex analysis of social determinants of health and their impact on resource utilization. Performs specialized treatment planning that recognizes psychosocial, psychiatric, and medical needs of patients within approved clinical privileges and scope of practice. Provides individual, group, and family psychotherapy and advanced level care management interventions used in the treatment of patients across the hospital's care continuum. Role model professional social work practice skills and ethical behaviors in all areas of service delivery. Demonstrates the ability to design changes based on empirical findings and utilization of outcome evaluations. Provides orientation and/or clinical supervision to less experienced social workers. Supports the MSW Training Program by supervising MSW Interns. Adheres to Medical Center Memorandums (HPM), Standard Operating Procedures (SOP) as evidenced in documentation. Independently provides psychosocial services throughout this high complexity facility during evenings, weekends, and holidays when access to supervisors is not readily available. Coverage is provided to complex patients located throughout the hospital including but not limited to the following inpatient and outpatient sections: Emergency Department, inpatient psychiatry, inpatient acute medicine, surgery, cardiology, outpatient clinics, rehabilitation medicine, etc. Services may include discharge planning, crisis intervention, screening, advanced directives, community referrals, responding to consults, and other psychosocial issues. Provides clinical supervision of other social work staff assigned to work evenings, weekends, and holidays. Reviews and signs off on progress notes for unlicensed social workers. May require meetings and evaluations to prepare social workers for licensure exams. Screens all referrals for Abuse and Neglect during evenings, weekends and holidays for appropriateness and accuracy and ensure that they are submitted per hospital policy. Work Schedule: 4 Days a week, 10 hours per day, with rotating tours of duty as needed. Recruitment Incentive (Sign-on Bonus): Not Authorized Pay: Competitive salary and regular salary increases. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-12 Senior Social Worker candidates must have the following: Experience/Education: The candidate must have at least two (2) years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one (1) year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: Acute care social work or emergency room experience in VHA. VHA Computer Patient Records System knowledge. Experience in crisis intervention, care coordination, care management, disposition, and discharge placement is required. Be able to work in a fast-paced environment due to high acuity, large patient volume, and rapid patient turnover. Essential to have the ability to effectively triage, complete a comprehensive needs assessment, and understand social determinants of health that can be managed on an outpatient basis. The role aims to prevent social admissions whenever possible. Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty.
