The Population Health Department at Union Health is looking for an Ambulatory Care Clinical Pharmacist for the Terre Haute campus. This role plays an integral role by managing the use of medications in hospital with goals include the selection, prescription, procuration, delivery, administration, and review of medications to optimize patient outcomes and assures that services are provided within professional standards and organizational policies to assist patients who lean on the Pharmacy team for answers.

    Ambulatory Care Clinical Pharmacist Essential Job Duties:
  • Performs drug information functions for patients, medical and ancillary departments, and pharmacy staff, utilizing Internet search engines and Lexicomp effectively; assists in training staff.

  • Conducts drug dosing consultations, demonstrating personal knowledge through consults and participating in PI activities for drug dosing services.

  • Serves on various committees assigned by pharmacy administration, such as P & T, Stroke, Infection Control, Medication Safety, Glucose Control, Pain Team, Sepsis Team, Antimicrobial Stewardship Team, Policy Committee, Readmissions.

  • Engages in pharmacy education activities, including planning staff development programs, providing educational programs to medical staff, nursing, and ancillary departments, and offering patient-oriented education.

  • Involvement in clinical PI/documentation activities, assisting in drug formulary management processes.

  • Focuses on professional development and competency assurance, attending educational offerings, observing health system policies, complying with state and national pharmacy laws, performing special projects, and exhibiting dependability.

  • Co-management of patients' chronic disease state(s) through collaborative drug therapy management (CDTM) protocols in conjunction with Union Medical Group (UMG) Primary Care Providers to ensure effective guideline directed use of medications.

  • Provide direct patient care in the area of anticoagulation management including point of care INR testing, vitals, and limited physical exams to determine patient response to therapy.

    Education, Training & Experience:

  • Advanced Pharmacy Degree (Pharm.D.)

  • Board Certification preferred in the first year of eligibility.

  • One to two years experience in ASHP accredited residency or equivalent experience preferred.

  • Ambulatory experience preferred.

  • Licensed or eligible for licensure in the State of Indiana. Must take licensure examination at the earliest convenience with licensure received within 75 days following first day of employment in position. Job duties of a graduate pharmacist may be performed under the supervision of a pharmacist. Failure to pass the licensure exam results in a change of status to Pharmacy Technician/Pharmacy Intern.

  • Licensed for the State of Illinois preferred.
