The All-Babies Initiative (ABI, www.allbabies.org) is a program for pregnant women, fathers, and babies up to 18 months old. ABI helps participants find resources in the community, provides education and support to families where most convenient by offering home visiting services. This position will work hand in hand with healthcare providers and community health workers to ensure UH PNP and Healthy Start compliance by creating and monitoring participant care plans and outreach to ensure social, financial, nutrition, and family wellness needs are met.

    Perinatal Navigators Essential Job Duties:

  • Manage a case load of enrolled participants with complex care plans, involving several team members based on identified needs.

  • Travel to client homes, clinics, community agencies, and other outreach destinations to more effectively reduce barriers between clients and institutions.

  • Build and maintain positive working relationships with clients, providers, nurse care managers, agency representatives, supervisors, office staff, from diverse cultural and socio-economic backgrounds.

  • Document and maintain timely, accurate, and thorough records of participant interactions, care coordination and compliance through WFS Case Management system and partner clinical medical record (when applicable).

  • Follow established guidelines and processes to provide consistent onboarding, support, and evidence-based patient education regarding perinatal topics and best practices throughout pregnancy until baby is 18 months old.

    Education, Training & Experience

  • Bachelor's Degree required in Family support services, Nursing, Psychology, or Social Work

  • Bilingual in Spanish and English strongly preferred

  • Outstanding verbal and written communication skills

  • Valid driver’s license required and working vehicle to successfully facilitate transportation to/from but not limited to home visits, community visits, and clinical sites.
