Job Overview:

The Occupational Therapist evaluates and implements treatments in collaboration with the multidisciplinary team.

Qualifications:

Required:

  • Graduate of an accredited Occupational Therapy Program

  • NJ License

  • BLS certification through the American Heart Association

  • Successful completion of all required orientation programs

Scheduling Requirements:

  • Per Diem

  • Day Shift

  • Weekends required

Essential Functions:

  • Coordinates and collaborates with healthcare team to provide the highest quality service possible

  • Communicates findings to Case Managers

  • Communicates and discusses cases with referring practitioners as needed

  • Consults on cases and issues providing professional advice to team

  • Instructs nursing and other team members how best to manage patients considering rehab needs

  • Coordinates care with other rehab disciplines

  • Determines the plan of care and the prognosis, predicting the optimal level of improvement in function and the amount of time needed to reach that level

  • Designs a plan of care that integrates goals, treatment, outcomes, and discharge plan

  • Appropriately creates patient centered goals and predicts outcomes and determines criteria for discharge

  • Establishes goals and desired functional outcomes that specify expected time durations

  • Establishes a plan of care in collaboration with the patient, family, caregiver, and others

  • Establishes a plan of care consistent with the examination and evaluation

  • Establishes a plan of care minimizing risk to the patient and those involved with the delivery of the patient's care

  • Demonstrates awareness of contraindications and precautions of treatment age specific

  • Establishes a plan of care designed to produce the maximum patient outcomes utilizing available resources

  • Adjusts the plan of care in response to changes in patient status

  • Identifies the most appropriate settings for service, explores all alternative service delivery options and effectively implements plans

  • Establishes the plan of care considering barriers to learning

  • Educates others patients, residents, family, caregivers, staff, students, other health care providers, using relevant and effective teaching methods

  • Designs educational activities to address identified needs

  • Conducts educational activities using a variety of instructional strategies as needed

  • Assesses effectiveness of educational activities

  • Evaluates findings, considers current level of impairments and probability of prolonged impairment, functional limitations and disabilities

  • Performs regular re-examinations of patient status

  • Performs regular evaluations of the effectiveness of patient treatment

  • Evaluates changes in patient status

  • Bills accurately and according to policies and regulations

  • Schedules patients/ residents to maximize treatment outcomes and efficiencies

  • Reports near misses and incidents to appropriate parties

  • Performs evaluations/ assessments and screenings including history, systems review, tests and measures

  • Obtains accurate information by performing the selected examination methods

  • Adjusts examination according to patient response

  • Performs examination minimizing risk to the patient, self, and others involved in the delivery of the patient's care

  • Performs therapy evaluation procedures in a technically competent manner

  • Spontaneously probes for additional relevant information, obtains info from other sources and integrates data in order to identify etiologic and or contributing factors

  • Administers evaluation in a uniform manner to ensure findings are valid and reliable

  • Selects reliable and valid examination methods relevant to the chief complaint, results of screening, and history of the patient/ resident

  • Provides skilled interventions, maximizing the patient's outcomes by successfully anticipating patients' response to care provided

  • Performs effective, efficient, fluid, and coordinated movement in providing technically competent interventions for patients

  • Performs interventions consistent with the plan of care

  • Determines time for discharge and educates patients on discharge responsibilities

  • Determines need for referral to other disciplines based upon progress, lack of progress with rehab care

  • Determines educational needs and educates patients' residents' families
