Qualifications:

• Age 21 and over preferred (18 minimum age)
• High School Diploma or GED
• Ability to successfully pass required background screenings
• Ability to successfully pass CPR/First Aid & DD Medication Assistance Course
• Ability to successfully pass WYTRANS Course
• Current driver's license with a motor vehicle record that is acceptable to Diversified Services Inc. insurance carrier
• Basic computer skills
• Knowledge of Therap

• Requires lifting, repositioning, transferring and supporting participants
• Ability to lift 50 lbs. or more

Essential Responsibilities:

• Maintains a clean and orderly residence for participants.
• Complies with all standards to assure the health and safety of all participants.
• Monitors the comfort and safety of participants. 
• Monitors/assists with medications of participants.
• Maintains accurate records, including documentation of medications, activities (schedules) and developmental progress of participants on Therap.
• Completes and returns all other required documentation to the Community Living Services Manager.
• Responsible for routine personal care of participants.
• Provides nutritious meals for participants.
• Accompany participants into the community.

Other Responsibilities:

• Works independently and with other staff members to ensure the Individual Support Plan (ISP) is followed.
• Adapts to the needs of the participant's demonstrating flexibility and adaptability.
• Attends trainings and meetings as required.
• Performs other duties as assigned.
• Covers scheduled shifts regardless of inclement weather and holidays.  (Community Living Services is 24-hour support of participants)
