Our Supported Housing program for those living with HIV/AIDS is looking for a Quality Assurance Coordinator to join the team.
This position will work closely with our SH team and program managers to ensure quality of care to those living with HIV/AIDS and funding compliance.
Responsibilities:
Work closely with the Supported Housing team to maintain quality care for clients.
Assist program managers and staff for those living with HIV/AIDS.
Data tracking and quality review.
Program documentation approval.
Competition of Eligibility Determinations.
Assist with grant/funding reports.
Ensure all compliance effort are met for funding.
Monitor and evaluate impact of services on Social Determinants of Health.
Promote an environment of inclusion and belonging.
Required:
Bachelor’s Degree in health, human, or educational services and three years qualifying experience or equivalent preferred. Extensive experience can substitute for a degree. Experience working with people living with HIV/AIDS or other at risk populations required. Valid and clean NYS driver license. Must have excellent organizational and time management skills. Prior experience working with in EHR systems and other databases a plus.
