Social Worker - Full Time
St. Peter's Health Partners (SPHP) is seeking a Social Worker to join our Ambulatory Care team. This full-time position will focus on providing clinical social work services, care planning, case management, and community resource coordination for patients and families in both inpatient and outpatient settings. You will work with a multidisciplinary team to improve patient outcomes, address social determinants of health, and connect patients to vital resources, including mental health and substance use disorder services.
Key Responsibilities:
Conduct assessments and provide clinical interventions to improve patients' physical, emotional, and cognitive well-being.
Utilize motivational interviewing and collaborate with healthcare providers to create integrated care plans.
Assist patients in navigating healthcare and community resources, focusing on mental health and substance use disorders.
Contribute to quality improvement (QI) initiatives and participate in the management of care within an Accountable Care Organization (ACO).
Support vulnerable patients in addressing social determinants of health and connecting to community-based services.
Requirements:
Master’s degree in Social Work (preferred).
NYS LMSW or LCSW certification (preferred).
Minimum of 3 years of clinical experience in a healthcare setting.
Supervisory experience (preferred).
Strong communication and team collaboration skills.

Pay range: $30-$43.08
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
