Nature of Work:

Incumbent is responsible for providing diabetes-related case management services and health education related to diabetes prevention and healthy lifestyles. Incumbent is responsible for overseeing the day-to-day operations of the Many Nations Community Room (MNCR) and kitchen, maintaining safety of clients and consistent cleanliness of MNCR and kitchen, assisting clients utilizing Community Room, and coordinating health promotion and diabetes education events in the Community Room. Additionally, the incumbent conducts program outreach and coordinates with other community health and social service agencies. Works with individuals and groups to facilitate their use of TIC and other services.

Essential Functions:

1. Provides individual case management services to clients addressing behavioral goals for a healthy lifestyle as relates to diabetes. Documents encounters in EHR.

2. Designs, delivers and documents educational activities for youth, adults and elders regarding aspects of a healthy lifestyle, inclusive of diet, exercise, and diabetes prevention. Documents encounters into the EHR.

3. Recruits and motivates urban Indian community members of all ages in weekly physical activity and health education.

4. Oversees and monitors the day-to-day operations of the Many Nations Community Room and the accompanying kitchen, maintaining safety, cleanliness, and standard business operating hours of 8:00 AM-5:00 PM.

5. Conducts community outreach to attract clients for health promotion services.

6. Develops educational and outreach materials that are appropriate for the Urban Indian community.

7. Refers individual clients to enroll them in Wellness Department services, to include client intake, eligibility determination, and case plan development and case plan monitoring.

8. Coordinates with TIC staff to coordinate the logistics for Community Room events, such as workshops and field trips.

9. Establishes and maintains working relationships with community organizations and partners to coordinate services.

10. Prepares required reports in an accurate and timely manner.

11. Participates as part of the staff team of the Tucson Indian Center and Wellness Department.

12. Participates in a professional development plan, as appropriate.

13. Performs other tasks as assigned.
