Exempt position

Position hours : Monday -Friday 40 hours per week: 8:00 am - 4:00pm

Pay rate: $29.00 per hour

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Become familiar with all the medical needs of individuals in the GDHs. Assess the need for physician visits. Act as a liaison to residential providers not physicians as needed or requested. Request copies of annual physicals, PPD, self-medication assessments, consults, and pertinent medical information from residential providers. Interpret medical reports and provide specific plans of care for the individual as required.

  • Perform assessments of nursing needs for each individual when necessary. Consult with applicable program staff regarding the nursing assessment. Provide training to individuals in health-related areas.

  • Participate in reviews and team meetings as requested. Provide advice and assist in the preparation, implementation and review of Habilitation Plans relative to medical concerns for certain individuals in conjunction with the program staff.

  • Establish and ensure maintenance of appropriate health care records either by program staff or by the nursing staff.

  • Provide on-going training to program staff in the areas of medication administration, preventative health measures and in areas of special individual need (i.e. post-operative, insulin, etc.). Conduct program observations to review implementation of medical policies and procedures or as required for medication administration certification. Provide feedback to the GDH management staff regarding employees' performance of medical procedures and plans of care for performance evaluations or as needed for monitoring and ongoing correction of medical procedures.

  • Supervise the medication administration practices. Teach medication administration certification and recertification courses. In conjunction with the GDH management staff

  • ensure all requirements are met for medication administration per OMRDD regulations.

  • Administer initial and annual PPD skin test to program staff.

  • Inform clinicians of changes in medical status and/ or medications for individuals on their caseloads.

  • Ensure the availability of appropriate first aid and nursing supplies at all GDH sites.

  • Participate in the Nursing On-Call rotation (weekly) and provide guidance as necessary through the on-call system to all residential program staff as needed.

  • Review and suggest revisions to the nursing, pharmacy and medical sections of the Program Policy and Procedure Manual.

    REQUIREMENTS:

  • Registered Nurse license by NYS Department of Education

  • Two years of prior nursing experience preferred

  • Experience working with individuals with a disability

  • Exceptional written, communication, and computer skills

  • A valid driver's license.

  • Ability to be certified in all required trainings

  • Occasionally required to lift up to 50 pounds
