Examples of Duties:

1) Conducts outreach to patients identified as needing care coordination and documents in patient record.

a) Promotion of preventive health care. b) Engage patients in their care to manage their chronic disease and establish routine PCP visits. c) Assess patients' medical and social determinants of health (SDOH) needs.

2) Facilitates collaboration and communication and documents in patient record.

a) Coordinates communication between patients, providers, and external organizations for continuity of care. b) Provides care coordination by utilizing appropriate resources within Grace Health and/or the community.

Licenses & Certifications:

Community health worker training and/or certification.

Technical Skills:

Demonstrates necessary proficiency with the electronic medical record system and the healthcare analytics and population health management system.
