Catholic Charities of the Archdiocese Case Manager in United States

Essential Functions of the position are detailed below and include any physical requirements below that

  • Provides comprehensive, ethnic specific case management and support services to new arrivals from various regions of the world during their period of adaptation to the United States, within their homes and with families in Madonna House Domestic Violence shelter

  • Conduct intake and assessment with new clients and prepare a resettlement plan with the goal of self-sufficiency. Monitors the client’s progress based on the services plan through home visits and phone contacts

  • Facilitates and coordinates agency and community resources including but not limited to healthcare provides, job readiness course, employers, ESL providers, school contacts, public assistance

  • Responsible for keeping financial records for each client. Assist clients in linking to other service providers

  • Participate in month case staffing with team at Madonna House and Migrant Response team to ensure all services are provided to clients in a timely manner to reach the goal of self-sufficiency

  • Foreign language proficiency to meet the needs of clients
