Role Responsibilities 1. Establishes, maintains, and grows community partnerships across East and West Garfield Park, Near Westside, and North Lawndale communities. 2. Promotes services provided at the Encompassing Center and also obtains information from partners about additional services needed in the identified communities. 3. Develop tools and systems to enhance community partnerships and lead projects involving community partners. 4. Responsible for the accuracy of all data entered into the applicable program database and ensuring that program metrics are met. 5. Follows an established process for referring clients for additional program services as appropriate and applicable. 6. Provides mobile partnership opportunities, including but not limited to outreach events, and meets tangible needs of the community through the distribution of care packages at partner sites. 7. Kneel and move from sitting, bending, kneeling, or standing multiple times a day. 2. Push and pull objects up to 10 pounds. 3. Climb up and down flights of stairs. 4. Lift up to 10 pounds.
