Mental Health Partners Homeless Day Center | Case Manager in United States

You will provide services at All Roads Day Program (formally, Boulder Shelter for the Homeless). You will connect individuals and provide coordination to outpatient services and resources to support ending the cycle of homelessness. Including, active participation on diverse, multi-agency/department treatment teams to assist clients in identifying, securing, and sustaining necessary resources to aid in their recovery. You will report to the Program Manager.

You Will:

  • Provide education and help strengthen daily living skills to support engagement in outpatient care and community resources.

  • Provide a variety of case management services, under supervision, assisting clients to apply for several types of public assistance and providing supporting documentation

  • Attend multi-agency clinical case consultations, and facilitating referrals to other organizations

  • Support client recovery by developing and enhancing client access to resources for vocational, financial, medical, housing, utilities, food, transportation or basic living needs

  • Facilitate groups, classes and workshops

  • Assist other teams with outreach, engagement, and paperwork

  • Participate in team meetings to staff cases and communicate administrative information

What we need for this job:

  • Bachelors in a human service field

  • Experience working with adults and/or children in an outpatient setting

  • Experience leading groups

  • Ability to work with a new program that is under development/demonstrate flexibility

  • QMAP training and certification will be required for some positions

  • Must have current CO Driver’s License and a safe driving record
