Registered Nurse - Case Management Location: Rapid City, SD Duration: 13 weeks Description: General Information Minimum Experience: 1.5 years general nursing (system requirement) Patient Types: Case Managers provide care for patients in all the acute care departments Patient Ratios: 1 CM to 16-20 patients Required Certifications: BLS Education Required: Completion of a nursing education program that is approved by a board of nursing; Registered Nurse (RN) - South Dakota Board of Nursing Skills required: Previous discharge planning and Acute Care Case Management experience, progression of care, experience meeting with families and discussing plans of care Support on the Unit: Case Management Assistants Lead Case Managers Social Work – Complex Psychosocial patient consultants Utilization Review RN’s Hospitalist Charting System: EPIC (required)
