MaineGeneral Health Registered Nurse (RN) - Home Health, Waterville, 32-40 hours/week in Waterville, Maine

Job Description Summary: MaineGeneral is seeking a dedicated Home Health Case Manager Registered Nurse (RN) to join our Augusta team. In this role, you will promote health, prevent illness, and manage the care of individuals with various medical conditions in the comfort of their homes. 

Job Description: $15,000 sign-on bonus potential for experienced RNs

  • Must have one year of RN experience and accept at least a 30-hour-per-week position.

  • Current MaineGeneral employees are ineligible; former MaineGeneral employees are ineligible until greater than 6 months of separation from employment.

The Work:

  • Collaborate with nursing staff and care team regularly to assess patient status and care decisions like admission, length of stay, transfer, and discharge.

  • Deliver direct treatment, education, and health condition management, advising providers on additional services and community referrals.

  • Develop, implement, and assess patient care plans based on assessments to improve outcomes. Monitor, record, and communicate patient condition as needed.

You Have:

  • An active Maine RN license or the ability to obtain the same in a timely fashion is required.

  • At least 1 year of acute or med/surg (or equivalent) nursing experience is required.

  • Previous ED, ICU, CCU, or home care experience preferred.

  • Valid driver’s license & acceptable driving record, required

  • Strong technology/computer skills and previous experience with electronic medical records (EMR) preferred.
