Overture Home Care offers RN-delegated home care and companion services to seniors and other individuals requiring care. The company's highly-trained caregivers are overseen by staff RNs, delivering a variety of services to enable their clients to live independently, safely and with dignity.

Duties include:

• Provide care as directed by the Case Manager to clients requiring in-home
• Provide assistance with ADLs, AIDLs, transportation, light housekeeping and meals
• Reports observations and other pertinent information related to the care of the client
• Documents information related to client care on agency forms or in electronic record
• Provides services that support the care delivered to the client
• Provides input in the formulation and evaluation of standards of care
• Participates in the development of client objectives
• Participates in the identification of staff educational needs
• Conducts self in a professional manner in compliance with agency policies
• Participates in the quality assessment and improvement process and activities
• Communicates clients progress to Case Manager; report condition changes
