We are seeking a compassionate and skilledSpeech-Language PathologistPRN (per diem) to join our team and contribute to the rehabilitation and well-being of our patients who are currently receiving our services in theWeatherford, TXand surrounding areas ofParker County.
The Speech-Language Pathologist will provide therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing, and cognitive disorders. This role involves planning, implementing, and evaluating patient care plans to restore or maintain patient well-being and participating in the coordination of care.
Requirements
Master’s or Doctoral Degree in Speech-Language Pathology from an accredited college or university.
Current State License in Speech-Language Pathology or meets the requirements listed in 42 CFR §484.115(n); current Driver’s License.
One year of experience in an acute care, rehabilitation setting, or home health preferred.
Therapy skills in line with generally accepted standards of practice; good interpersonal skills; proof of current CPR.
Reliable transportation and valid auto liability insurance.
Perform ongoing clinical assessments, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and assess appropriateness for home care.
Develop and evaluate the plan of care in partnership with the patient, their representative, and caregivers.
Prepare and complete clinical notes, phone orders, and other clinical record documentation in a timely manner.
Analyze patient needs and provide appropriate care as ordered by the physician, revising the plan of care based on ongoing assessments.
Provide therapeutic treatments for speech, language, cognitive, and swallowing disorders using appropriate tests and measurements.
Apply infection control and universal precautions to prevent contamination and transmission of disease.
Accept clinical assignments consistent with education and competence and provide supervision and guidance to home health aides as appropriate.
Communicate with all physicians involved in the plan of care and other health care practitioners as necessary.
Provide patient, caregiver, and family counseling and education.
Utilize effective interpersonal relations and communication skills, facilitating the use of these skills by other team members to achieve desirable outcomes.
Meet mandatory continuing education requirements and participate in the Agency’s QAPI program and in-service training.
Ability to work a flexible schedule and travel locally.
