Job Description: Provide health maintenance for patients and ensure Standard of Care for the patient within the Internal Medicine Group. Physician analyze and interpret data, formulated diagnosis and problem lists, and establish plans for the management of common and complex adult health problems. Treat problems with their scope of competence and exercise judgment on problems requiring consultation, referral or evaluation. Provide patient education in linguistic, appropriate manner on health matters pertaining to the patient. Manage common and complex chronic adult major illness. Responsible for the overall clinical management of all Adult/Family medicine medical providers and monitoring of the practices on site programs in order to meet the organization’s goals, quality performance measures, regulatory compliance, training programs, and research initiatives. Precept and collaborate with other staff members/disciplines to improve the patient experience. Participates in team meetings and activities of PCMH (Primary Care Medical Home) directly or indirectly. Enter into Joint Collaboration Agreements with one or more NHCAC Advance Practice Nurses. Enter into any other agreements as requested by the Chief Medical Officer that is reasonably related to the performance of the director’s duties and responsibilities for NHCAC. Conduct peer and performance reviews as assigned. Monitors and Manages Site Quality Performance.Job Requirements: Must possess MD in Medicine and 3 years of Medical Residency Training. Must also possess active Medical License in the State of New Jersey; Board Certification in Internal Medicine.
